WEST HAVEN, April 1, 2021 — Connecticut residents 16 and older can now register for the COVID-19 vaccine by filling out an online form with the Vaccine Administration Management System (VAMS), a portal administered by the state Department of Public Health.You will need to enter your full name, birthdate, ZIP code, occupation (including “retired”) and email address. The information you enter will only be used for purposes of scheduling your vaccination and will be kept private.Once registered, VAMS will email and guide you through the appointment process for receiving the vaccine. At this moment, only people eligible under Phase 1a, Phase 1b or Phase 1c who are 16 and older can register. Please only fill out the form if you are eligible to receive the vaccine.Please note that every individual must have their own unique email to be vaccinated with VAMS. For example, if you are registering your elderly parent and use your email, you will not be able to be vaccinated using that same email.Also, state residents 16 and older can now register for the COVID-19 vaccine by phone by calling the Connecticut Vaccine Appointment Assist Line at 877-918-2224.The line will take calls from 8 a.m. to 8 p.m. seven days a week and will offer a call-back option when all contact specialists are busy serving other callers. The team will aim to return calls as soon as possible, with the goal of same-day response.Please note that the Pfizer vaccine is approved for individuals 16 and older and the Moderna and Johnson & Johnson vaccines are approved for individuals 18 and older.
